Nurse-led Complex Care
Altair Healthcare offers nursing care or complex care, a home-based package of care delivered by qualified, experienced nurses who are registered with the Nursing and Midwifery Council, and specially trained carers to support service users with certain healthcare needs, to enable them to live independently at home.
This enables the person living with a complex health condition to continue living at home, without the need to move into a nursing or residential home, retaining their independence and stability.
Our Complex Care nursing teams are made up of registered nurses and professional care workers who are directly employed, trained, and supervised by us.
The teams provide specialist support to people with serious or terminal illnesses requiring nursing or palliative care including, but not limited to - continence and stoma care, PEG feeding, assisted transfers and hoists, tracheostomies and ventilated care - as well as the activities of daily living involved in home care.
We provide specialist packages of care/support for clients with complex needs and requiring specialist support for healthcare needs they may have. This specialist support is overseen and delivered by qualified clinical nurses and specially trained care workers.
Altair Healthcare works in partnership with Clinical Commissioning Groups (CCG) and Continuing Healthcare (CHC) teams in your local area to arrange packages of complex care that are fully or partly funded by the NHS.
Once the package of care is agreed, we also work with other health professionals such as your GP, district nurses and social workers to ensure a seamless care plan is in place to meet your health requirements.
