Community Mental Health Support
Who We Support
Altair Healthcare provides care and treatment to people living with mental health problems who need support in their own homes or in the community. Such support may be short or long term, and meet acute or chronic healthcare needs. We deliver safe, effective and excellent care to our clients in the comfort of their own homes, making them feel safe and well supported.
How We Support Our Clients
We will provide you with a personal support plan, developed with you at the start of the package, identifying the type of support needed in different areas of your life, and mapping out how we will help you to achieve these personal goals.
We will help you achieve goals that include:
- Maintaining your home and your tenancy
- Ensuring that your home is safe and habitable
- Budgeting and managing personal finances and benefits
- Learning or developing domestic and life skills like cooking, cleaning and shopping
- Connecting you with a wide range of community services
- Help to access employment, training or volunteering opportunities
- Reducing social isolation - help maintaining personal relationships, including keeping in touch with friends and family.
- Supervising you to take your medicine that has been prescribed by your doctor.
- Providing you with emotional support
- Facilitating communication with healthcare professionals, other agencies, family and friends.
- Communicating with the Community Mental Health Team or Home Treatment Team if client's mental health deteriorates
