Altair Healthcare provides specialist, nurse-led complex care services designed to support people with high-level clinical and personal care needs to live safely, with dignity, and with greater independence - in the comfort of their own homes. Our services are delivered by highly skilled nurses and care professionals, working in partnership with individuals, families, commissioners, and multidisciplinary teams.

What we offer:

We specialise in complex, regulated care and support for people whose needs cannot be met by standard domiciliary care alone. Our focus is on delivering safe, effective, and responsive care in the most appropriate setting – most often the individual’s own home.

We work in collaboration with community mental health teams to deliver excellent integrated, recovery focused and evidence based care in an environment that makes an individual feel safe and well supported.

Complex Care Expertise

  • Acquired brain injury (ABI) and traumatic brain injury (TBI)

Our Service Aims

Altair Healthcare is committed to delivering complex care services that achieve meaningful outcomes for the people we support. Our service aims are to ensure that:

  • People are able to remain in their own home for as long as they feel safe and supported to do so.
  • Premature or inappropriate admission or re-admission to hospital or residential care settings is prevented wherever possible.
  • Reliance on unnecessary state-funded health and social care services is reduced or removed through effective, proactive support.
  • The service is sufficient to meet eligible needs and anticipated demand from vulnerable people requiring regulated care and support.
  • Timely and reliable access to personal care and clinical support is provided, particularly – though not exclusively – in hospital discharge and step-down scenarios.

All services delivered are of good or excellent quality, as assessed and assured by the Care Quality Commission (CQC) and relevant commissioning and quality monitoring teams.

Autonomy and Choice

Altair will work with the client to create a support plan to meet their needs and manage their risks, and the client will have a dedicated support worker responsible for coordinating the client’s care and support.

Our team will provide a wide range of support, including:

  • Risk and needs assessment.
  • Care planning and evaluation.
  • Managements of risks, to ensure the individual’s safety.
  • Medication and medication management, and assessing effectiveness.
  • Supporting the individual to engage in mental health treatment programmes
  • Support to develop or regain life skills to enable independence e.g. cooking, shopping and performing other household tasks
  • Engaging in activities that build motivation, confidence, and self-esteem
  • Support to access education, training, volunteering or employment opportunities
  • Support to access community activities to promote social inclusion.

  • 24-hour live-in care packages providing continuity, safety, and personalised support within the home environment.
  • Complex Waking Night Support / Night Care - Overnight waking support to meet clinical, safety, or behavioural needs during night-time hours.
  • Complex Sleep-in Night Care - Sleep-in arrangements providing reassurance and responsive support where full waking nights are not required.
  • Complex 24-Hour Care Fully managed, round-the-clock care packages combining nursing, personal care, and specialist support.

Flexible Care Settings

Altair Healthcare services can be delivered across a range of settings, depending on assessed need and commissioning arrangements:

  • In an individual’s own home
  • In temporary or short-term break accommodation (respite care)
  • As part of an agreed short-term break or rehabilitation package
  • In exceptional circumstances, as additional temporary care and support within a care home to supplement existing provision

This flexibility allows us to respond quickly to changing needs, discharge pathways, and risk-managed transitions.
